Exchange Transport Rule - Warning displayed when Outside organization - Microsoft Community

hi

i have transport rule set display caution warning when email received sender outside of organization , there attachment > 1kb. 

it works efficiently, on (not all) emails it's shows when sender outside of organization there no attachment email?  curious on why displaying. outlook consider aspects of body attachment? 

does have idea?

we signed atp licenses, , not longer using warning.
